She limped in the room with a cane, beautiful deep dark skin, high cheek bones with large eyes, coved with a tint of redness, possibility from crying. She sat confidently. She could have been an African royalty by her mannerism.

Good afternoon, welcome to the clinic, how can I help you? I am here because for my hip pain, back and neck pain. I use this cane, without it I am afraid to fall.  What happened that you ended up with this hip pain and a cane?

I slipped and fell off a flight of stairs. I work in East LA school district as a Special Education Teachers Assistant. We see kids that are in gangs or are academically behind. There are these Hispanic kids in my school who were harassing me and calling me Monkey, and make monkey sounds when I pass by,… her tears rolled down her check landing on her shirt, she gracefully and bashfully took her fingers and gently wiped her tears away to avoid her mascara covered her skin.

My jaw dropped! That was not what I expected to hear in 2018, in America! WTF! We had a black president, we are in a democratic state, we host immigrants from all over the world. She is an educated, respected 50-some year old woman. How could this happen? We have systems, laws, regulations.(What in the world!!!!! Is this behavior all about, from another minority)?

You see, I am from Panama, the English speaking side, the Spanish gang in my East LA school , 4 boys, from 14-16, think I am black, they harassed me calling me names and bullied me in hallway as I was passing, I got nervous and scared wanted to get away I rushed towards the stairs leading to faculty room, I missed the first step, fell off 8 steps landing on my hands and side. One of the teachers who was going to her class saw me and came to rescue. Embarrassed scared, and in pain, I put myself together and walked away hoping walking will take away the pain.

Took few days off, I contemplated to file for work injury but am so scared of losing my income. So I decided to be quiet, just stay in during breaks and lunch to avoid running into to them in the school yard.

I listened in disbelief and was heartbroken for her. All I can do is just do my best for her. “Sometimes its not doing the right thing that matters its doing what is the best to be done given the circumstances you are faced with.” She said while holding back the tears.

Sharleen, I reached out to her and held her hand. I will do my best to take care of your pain, that way, you can make the best decision for you and your family when you feel healthy Ok? She managed a smile and in her Caribbean accent said “that would be a good”.

So what area of your body bothers you most that you want us to address first together?

I have terrible hip pain [Fig-1] without this cane I cannot move around, the pain is too terrible. It sometimes radiates to my back or front of my leg. Its worst when I sleep on it or sit for too long, stand in line at supermarket. I warm it up at home by the pain is unbearable. I have to hold on to the stairs to walk up/down. Its there all the time and gets worse or better thru out the day.

My examination was to find the underlying cause and prioritizing her condition based on her examination and response to tests. [Fig-2] Trendelenberg test: is (+) for weak gluteus muscle, lack of stability in walking is consistent with her pain pattern. She feels sever pain with palpating the area around her hip and worse with any resistive static hip muscle tests. Active and passive ranges of motion (ROM) were limited overall by 50-80%, but it was the resistive ROM was extremely painful.

Xrays: 2 view Hip, 2view Lumbo-Pelvic, (Seated and Standing AP-Pelvis). & 4 view L/S. These are dynamic x-rays to show how your body is compensating or has learned to adapt to damages and your limp.

Along with your exam findings and x-rays, its easier to find the underlying cause of your problem to help determine if your problem is soft-tissue or hard tissue (muscular vs. skeletal). From there we can ascertain the best course of care for you.

 

  • Soft tissue treatments
  • Phase I: mild myofacial release and P-ROM, PhaseII: deep tissue and –ART, if at any time you feel worse we need to bring in medical support for Trigger point Therapy (TPT) with injection if necessary.
  • So this is a 2 part problem:
  • Pelvis tilts with every step upward and forward. The forward tilt can cause “Femoral Acetabular Impingement” and groin pain.
  • Pelvis unstable due to muscle weakness. Muscles/Tendons/Bursa inflamed due to above. Note that until the muscles are strong this will continue to be a problem. The exercises may be painful or cause pain but they will continue to contribute to the problem until they are strong so its important to use OTC [over the counter] pain relief to get you thru the acute while we work towards goal of Phase I care program: (To reduce pain and inflammation as a tool to get the muscles strong).

Hip traction, Ultrasound and heat along with spinal alignment and trigger point care will help you perform your exercises as below while in office: GLUTEUS MEDIUS STRENGTHENING – 45 degree HIP ABDUCTION STRENGTHENING STANDING or SIDE LYING recommended 4x per day sets of 10-15 reps.

  • By 4th week she was 50% better, and was diligent with her home care.
  • By 12 week she was pain free, Cain free and back on her feet walking without a limp. She would need to be proactive with her health and join swimming classes or aqua aerobics. She mentioned she has been stronger than before physically and emotionally.

Most kids today prefer to use backpacks to carry their stuff to school. These bags allow the kids to carry weight on two shoulders instead of just one, and they also leave the hands free for other stuff like smartphones. Also, most kids just consider them cool, especially compared to the alternatives.

But what’s not cool at all about backpacks is that they can actually cause some harm to your kids.  In 2001, emergency rooms across the US saw a total of more than 6,000 cases of injuries to children related to carrying a backpack.

A lot of the problem has to do with overloading. A recommended weight of a backpack  should not exceeds more than 10% to 15% of the child’s weight. But about a fourth of all young students bear more than 20% of their body weight on their backpack.

For example if your child weigh’s 80 lbs the weight of his backpack should not exceed 10-15lbs Students body weight X 0.10 = total weight the body can handle in the bag.

Here are some Exercises to Counteract Constant Backpack Weight

According to some experts, the best exercises to counter the effects of backpack use involve strengthening the muscles involved in carrying a backpack.

  • Exercises to strengthen the abdominal muscles should be done about three times a week. One example is the crunches on a medicine ball, and two sets of 8 to 12 crunches are recommended.

Another good exercise is the bicycle maneuver. You lie down on your back with your legs outstretched and your hands behind your head. Then pretend that you are riding a bicycle by alternately bringing your right knee towards your left elbow and then your left knee towards your right elbow. Two sets of 10 to 12 repetitions should suffice.

  • Lower back exercises should also be done. One exercise has you lying down on your stomach, while you tighten one leg and raise it a few inches off the ground for 10 seconds. Do the same thing for the other leg? You can do the exercise about five times.
  • Finally, the upper back and shoulders should also be strengthened. Pushups are good examples.

To encourage your kids to do these exercises, maybe you can do the exercises with your kids. It will help keep you healthy as well.

I received an email today asking me…  Better Posture

I have a hump on my back how do it get rid of it and my poor posture?  In fact just this afternoon, I had a female patient who said her hump is already lowered after wearing BAX-U POSTURE SUPPORT for 1 week. wow! 
So how dose a hump in your back appears?
Your body is composed of 3 major functional curves: Neck (cervical), Mid-back (Thoracic), and Lower back (Lumbar). Case 1: Sera works at DWP.

“Neck pain radiating to my arms.”

Sera began, “Working at DWP is not as easy as it seems. Living 6-8 hrs/day in a cubicle surrounded by photos and souvenirs to remind you why you are there is how I spend my days. I sit in front of computer for  hours at a time. A coffee break to Starbucks downstairs is a habitual pattern even when I don’t really want any coffee! I think I am more addicted to coffee than a smoker to cigarettes, and somehow like a zombie I just want starbucks! Is this a marketing conspiracy trap? I wonder at times, justified by the delicious sandwiches and little pastry bites. Mmmmh I wonder”.

Sera was talking out loud sharing her thoughts without filter.  I was on the clock and she was venting,…. Okay get to the point I thought, I am not a psychologist, nor do I want to be.

“Gott’a take the rims on this one,…” I smiled and said to Sera, “ Sera why are you here? And how can I, as your chiropractor help you?” I smiled and hoped for a direct response.

“Oh, oh sorry, its my neck! I have this knot that is not going away my massage therapist at my massage place said I should see a chiropractor for it, I have spent over 1000’s of dollars and still have pain!. The pain gets so bad that it shoots in between my shoulder blades and sometimes I feel it run through back of my arm to my elbow”.

My eyes were wide open and my ears alert: “Wow, on scale of 1-10 (10 being the worst pain) how would you rate this pain?”

“5 all the time in my neck but occasional sharp 8/9 in between my shoulder blades and it lasts few seconds to few minutes then it’s gone”. She pointed to the area, “I wake up with sharp pain if I move a certain way”.

Ok! Let’s dig in and find the cause,… (I love this investigation game,… lets find it and fix it) …. Sera how long have you had this and when did it first start to bother you?”

“It started few years in to my work, you know I run payroll for DWP, so I am crunching numbers, typing or on the phone, all day. It wasn’t ‘til my fall at ice skating rink! It has been downhill since a year ago. I even have occasional headaches and I NEVER had headaches before.”

“Oh my, that is not good.  How has this problem affected your life at work or at home ?”

“I can’t sleep at night without tossing and turning. I spend more money on coffee and massages but I still have pain. I tend to lose patience with my kids and driving over bumps feels like a sharp stabbing pain in middle of my neck. I have become clumsy and am dropping things too,  I broke 2 cups last week. It is not like me to do that.”

“Well based on what you are saying seems like you have nerve pinches in your Neck area that is affecting the sharp pain between your shoulder blades, leading to compensatory misalignments in the areas above and below the source in your neck.  These symptoms you mention are very typical of C5, C6 nerve pinch and your grip strength comes from your C6 area. Any weakness in that nerve flow till affect the integrity of the muscles it innervates, specifically your thumb and index finger grips.

In your exam I will perform a couple of tests to investigate the underlying cause of your problems and figure out the best course of action for your care. If you are a candidate for my care or not will be discovered after these exams, Shall we start?”

Neurological to check nerve pinch or compression at sensory or motor levels.

Orthopedic tests help, to see if it’s a disc problem or bones spurs causing you pain.  Chiropractic exam will be your Range of motions and muscle Palpation checking to see how far from normal ranges have you gone.

I am looking to find the underlying CAUSE of your problem and help resolve the cause.

Should you have positive findings I will recommend taking X-rays, to determine best  treatment plan suited for you. Any questions?”

“Wow doctor my other chiropractors never looked at me like this.” Sera stated.

“Well this is what I do here, I do not want to waste your time or my reputations if I can help you, I’d be happy to and if I cant, I would refer you to one who can.   You are ready?”

The examination discovered a long list of muscle damage, skeletal torsion and poor posture, lack of joint movements, and disc insults but her DRT & Sensory was intact,… let’s look under the hood!

Sera, looked at me at with an awed expression, I am always fascinated by how simple, natural healing, is such mystery to people. I think she would be the kind of patient who is willing to learn and make a difference in her future. Off to X-ray she goes, let’s see what she has going on.

Report of Findings day: Sera came back with her boyfriend. They sat in the waiting room. How lovely is that to have her support with her, good for him.   Sera’s x-rays are up, you have reversed cervical curve causing you to have reduced Disc size at C4-C7 most significantly at C5/C6 area causing Nerve root compression..

Aggravated  by: texting, or poor sitting positions at work while using your work computer, driving too long or sleeping on several pillows makes your condition worse. Subject to early degenerative arthritis which at 25 is not what you want!

This has caused development of minor bone spur which if left uncorrected will lead into a more serious condition and subject to surgery.

Your guide to improve your posture(and back health)

When your posture is in correct position, your spine becomes straighter while standing, sitting, walking or even lifting any object. That is spine at its optimal state. So what is OPTIMAL POSTURE? A smart posture is having your head up looking towards horizon, shoulder relaxed and pulled back, chin up, your chest expanded, stomach pulled in and an overall sense of better breathing and power, confidence, joy and well being. Practicing the correct way to stand, sit and walk will boost your mental and physical health. Which body physics can offer better decision making in daily life,… Taller, Broader Shoulders, Chin up, OR Shorter, Belly out, Slumped shoulders?

Slouching is bad for your back health. Why you would ask,… everyone does it. Well good point to share this article with the “everyone” you know. Your spine is the Tunnel of communication between every cell, tissue organ and system of your body. Does it benefit you to ignore Poor Posture physics?

Here are some helpful tips in improving posture:

1- Movements activities to correct your back. Keep changing your positions more often when sitting or standing over 30min. Remaining in the same position will strain your body and limit the range of your daily activities.

2- Check your chair. Make adjustment to your chair in a way that your foot touches the floor. Never cross your legs above the knees as this too causes poor circulation. [ see Ergonomics 101 video]

3-Take small breaks every half hour, go out, stand or walk for a while and then come back to your working position. It will relax your body muscles. Added benefit is this action helps your nerves when angry, confused, uncertain, or sad. (works well in relationship arguments).

4- Have a firm footing while lifting an object, for this way you save your back. Keep your spine straight, your chest out and bend at your knees and hips.

5- For better bone density take a walk for 20min /day with some sort of light weight. For added benefits, to boost metabolism, take these walks early morning or late in the evening.

6-Keeping your weight normal can help you maintain a healthy spine. Try an APPLE DAY, one day of the week eat very light or only apple and water. [ this is a suggestion not a prescription for treatment/diagnosis. check with your doctor first.]. Keep changing your diet chart. Fasting once a week will also help you relax and rejuvenate from within.

7- Check your body weight on a daily basis.

8- Good exposure to sunlight and foods rich in Vitamin D is beneficial for strong bones and muscles.

9-Have some orange juice, eggs, milk, mushrooms, and cod liver oil as these are some of the richest sources of Vitamin D. [Before you start with any health supplement, consult a doctor].

Now fast forward an adventurous historical 30+ years of changes in the world, and Me.   Today I am a trained Doctor form America, and see people in a different view.   I see more people with their shoulders rolled in and heads down.   Except it’s not for being graciously humble, it’s for different reason.  As the world speeds thru Smart high tech devices.   Our bodies & behaviors change to meet the demands imposed upon it.

Do you text? use computers? drive?

Body talks:

As you lean into your computers, your “Shoulders rolled in, back is slouched, and  head moves forward”.   This unconscious, simple, adaptive, change can affect the nerve structures feeding to many key organs. Insulting the harmonious melody of form and function of the body.  You get DIS- EASE State of health and symptoms occur such as PAIN.

Mind Reacts:

Forward head position, can affect C5, nerves which feed to Vocal cords, shoulders and diaphragm. In Buddhism the 5th chakra/Throat Chakra, is the ability to clearly communicate ones feelings and thoughts.

This compression with 5th chakra & C5 nerve, can have behavior effects,… One can become less talkative, more introverted, might feel exhausted.  This graceful, humble posture I was taught can actually affect more than a nerve pinch and neck pain.   Affecting ones, ability to clearly communicate, hold her/his stance, grant themselves freedom of self expression, speaking ones mind, opinion or truth. I can say in summary, its freedom of self expression.

Spirit Manifests:

Confidence, Belief, Certainty, Happiness, Love and Joy are spiritual manifestations in optimal expression when body and mind are at their best. Poor posture affects mind and body, it can affect life in decision making, out look to future, confidence and trust in self and others ,…

In my clinic this is what I teach my patients to screen their families and self.  Ear lobe to shoulder, test.  

 In the world, the integration of social media, and keeping up with change is our daily life style.
